Yes. WorkSafe manages personal and health information in accordance with the:

• Privacy and Data Protection Act 2014 (Vic) and its Information Privacy Principles.
• Health Records Act 2001 (Vic) and its Health Privacy Principles. This means that all personal information that WorkSafe collects and stores is protected, and will not be shared without your explicit consent to do so.

We ask you if it's okay to pass on your survey answers to your agent and let them know they are from you. We also ask if they can contact you to discuss your feedback. This can help improve the way they deal with other injured workers. If you prefer to remain anonymous, you can still agree to have your answers identified by the WorkSafe Research Team. With this option, your agent will receive your feedback but they will not know it's from you.

The purpose of this survey is to get feedback from injured workers about their experiences following an injury. With a better understanding of their views, WorkSafe Victoria will be in a better position to meet the needs of other injured workers in the future.

Up to 5 minutes, depending on your answers. There are typically 3 to 5 questions per survey.

No, it's your choice. Participation is completely voluntary.
