What it contains

Under occupational health and safety, dangerous goods and equipment (public safety) laws, an employer or employee can request that WorkSafe review an inspector's decision. This document is a form for applicants to request a review. Applicants can use the form to explain:

• under which act they would like they decision reviewed
• how they need an extension of time for an improvement notice
• why they want a decision changed
