Health and safety representative request to attend training form

What it contains

Sections 67 and 69 of the Occupational Health and Safety (OHS) Act outline the entitlements of HSRs to attend training for their roles. This training should be WorkSafe-approved and helps HSRs gain the skills needed to perform their roles and represent members of their designated work group. This document is a form for HSRs to get approval from their employer to attend:

• initial OHS training
• refresher course training
