Driver commercial health assessment – medical certificate

What it contains

Drivers wanting to apply to WorkSafe for a dangerous goods driver licence or a licence to drive a vehicle transporting explosives by road should use this form.

You will need to fill it out and have it signed by a registered medical practitioner. WorkSafe will use the form to decide whether you’re medically suitable for either of these licences.
