Application for exemption

What it contains

This is a form to request an exemption from complying with OHS Regulations. The form requires you to provide information about:

• the scope of the exemption
• a description of the process, substance, activity or item the workplace is requesting be exempt under the regulations
• reason for the exemption request
• how the workplace has consulted about the request
• how the workplace would prevent health and safety risks if the exemption was approved
