Complete this form if you wish to consent to:

  • National police check
    A national police check for WorkSafe to obtain known information concerning you. This is to make sure you satisfy the requirements to hold a licence or permit. The check is done to meet a requirement of the Dangerous Goods Act 1985, as well as the Dangerous Goods (Storage and Handling) Regulations 2022, the Dangerous Goods (Transport by Road and Rail) Regulations 2018, the Dangerous Goods (HCDG) Regulations 2016 and the Dangerous Goods (Explosives) Regulations 2022 (the regulations).

    You will need to consent to a national police check every time you apply for, amend or renew your licence.

  • Australian Security Intelligence Organisation (ASIO) security assessment 
    ASIO conducts security assessments across a range of areas and provides WorkSafe with advice about whether an individual poses a security risk, and whether action should be taken.

    Under the regulations, an applicant must consent to an ASIO security assessment. An ASIO security assessment is for life, so you have to provide consent to WorkSafe only once.

    If you believe you have completed an ASIO security assessment for another WorkSafe licence/permit application, then you do not have to complete it with this application. If WorkSafe's records show that you have not consented to this check in the past, then we will contact you by email. You will need to complete this form and pay the fee before your licence/permit can be issued.
