Use this form to become registered with WorkSafe Victoria to provide mental health occupational therapy services to injured workers.

1. This form is to be used for all mental health occupational therapist applications.
2. All sections of the application form must be completed and all applicant declarations signed.
3. Only application forms from eligible applicants will be assessed against the Assessment Criteria.
