Non-emergency patient transport providers
Non-emergency patient transport prroviders must hold a current licence to provide non-emergency transport services through the Department of Health.
Applicants must provide documentation that will confirm they have a current licence as a non-patient transport provider through Department of Health.
Application process:
- complete the WorkSafe application for registration to provide services to workers form
- sign the provider consent and declaration on the above form to acknowledge their experience satisfies industry requirements
- provide documentation you have a current licence to provide non-emergency transport services with Department of Health
- WorkSafe will issue applicants with a provider number for invoicing purposes
Private and public ambulance
Private and public ambulance providers must hold a current licence to provide ambulance services through the Department of Health or Ambulance Victoria or interstate equivalent.
Applicants must provide documentation that will confirm they are accredited to provide ambulance services through Department of Health or Ambulance Victoria or interstate equivalent.
Application process:
- complete the WorkSafe application for registration to provide services to workers form
- sign the provider consent and declaration on the above form to acknowledge their experience satisfies industry requirements
- provide documentation you have a current licence to provide public and/or private ambulance services with Department of Health or Ambulance Victoria or interstate equivalent
- WorkSafe will issue applicants with a provider number for invoicing purposes
