Informal Release of Information (IROI) request

Important notes:

  • The IROI program is limited to releasing the following documents, subject to eligibility:

    • Inspection reports
    • Improvement notices
    • Non-disturbance notices
    • Prohibition notices
    • Provisional improvement notices
    • Provisional improvement notice enquiry outcome notices
    • Virtual enquiry activity reports

    No other information or documents will be considered for release under this program.

  • Documents released through this process will be no older than 12 months at the time of the request. If you would like to request documents older than 12 months, or documents outside of those listed above, you may do so by submitting a Freedom of Information (FOI) request.
  • A separate application is required for each WorkSafe inspector site visit, unless multiple site visits relate to the same health and safety issue.

Your request will be considered in accordance with the Occupational Health and Safety Act 2004 and all reasonable steps will be taken to ensure that applicants are notified of a decision as soon as practicable.

Send an email to [email protected] with the following information to lodge a valid IROI request, without this information your request may not be accepted. You may also send this information to the IROI team by mail using the mailing address above.

  • Name of the applicant:
  • Contact phone number of the applicant:
  • Return email address:
  • The capacity of the applicant
    • Authorised representative of an employee organisation
    • A representative of an organisation representing employers
    • A representative of an organisation representing employees
    • A person whose interests are affected by the information to which they seek access
  • The purpose of the request
  • How the request relates to health and safety
  • The date of the site visit by the inspector
  • The name and address of the workplace you are requesting information about
  • Description of the information you are requesting
