About the Support Coordination Program
A two-year initiative aiming to improve support for injured workers with complex needs.
Introduction
The Support Coordination Program (the Program) is a two-year initiative aiming to improve support for injured workers with complex needs and their experience of claims management.
Every injury is different which is why we’re offering a tailored approach to injured workers to meet their diverse needs. The Program aims to help injured workers achieve their individual goals at a pace that works for them.
Two well-established providers of support coordination; Melbourne City Mission and Star Health are participating in the Program. Both providers have significant experience in supporting people with complex psychosocial needs.
The Support Coordinators (SCs) work with injured workers, their families and their treating teams to develop and implement person-centred plans. Injured workers participating in the Program will be the decision makers in their own recovery.
The primary aim of this Program is to enable optimal independence, functioning and participation relevant to the injured worker, by:
Optimising the injured worker’s abilities so they can live their best life, by:
- interacting with their community
- identifying options for participation - eg social groups, education options, entertainment, leisure activities, group exercises
- considering any barriers and whether/how they can be overcome
- establishing meaningful and purposeful activity
- exploring values and goals with the injured worker - eg past activities and interests, previous work roles, new areas of interest
- considering any barriers and whether/how they can be overcome
- recognising actions or inactions that may create risk to for the injured worker
- interacting with their community
Assisting the injured worker to determine when clinical management is required and when they can self-manage symptoms and flares, by:
- talking together with a trusted clinician
- learning together about the injured worker’s condition and evidence based management of this, by accessing high quality resources such as the Better Health Channel, the Cochrane Library, Beyond Blue, Black Dog Institute, Painaustralia, Musculoskeletal Australia
- reflecting back on past experiences
- considering together any barriers and whether/how they can be overcome
